Lately, ( since the beginning of Feb ) almost every website I visit issues this popup asking me to participate in a survey of my usage, and offering a "valuable" gift for taking part. This usually occurs after I click on a link to a secondary page for the site. I have tried taking the survey, hoping that the popups will then quit. No soap! They still activate every time. One site I use will generate the popup several times as I traverse different links on the site. Of course I have blocking set in the CONTENTS section of the TOOLS menu.

You have probably accidentally installed a slightly malicious add-on. Which is creating this particular pop-up.

I have had this issue before in the past.

to remove it. You need to find the add-on which is creating them and remove it.

to remove add-ons follow these instructions https://support.mozilla.org/en-US/kb/disable-or-remove-add-ons

Thanks, Nick! It never occurred to me that some less than reputable site stuck me with a stealth add-on. FYI the offending culprit is somebody calling themselves "Coupon Server". Look for it at a browser near you...
