I continually get requests from http://myrewardssurveybrand.com/ to take a survey. I don't want to take any surveys and certainly not from a company asking me to take a survey every time I open my browser

It sounds like you may have malware. Following these steps should get rid of it:
- Troubleshoot Firefox issues caused by malware
- Refresh Firefox - reset add-ons and settings
- Install www.adblockplus.org
