Firefox still plays sounds in muted tabs

If I rightclick a tab and choose "Remove tab's sound", it will make it silent and stop playing sounds. But sometimes when I go back to the tab, it starts playing sounds again, like music, even when muted. This is very inconvenient, and shouldn't happen, even if my Firefox is somehow corrupted and I didn't notice. It keeps playing sounds until I unmute and re-mute the tab!

Did you click on the tab or the speaker on the tab when you clicked the tab back?

No, I think I used alt + tab. I also didn't accidentally uncheck "muted", the "muted" icon was still there, and it was playing sound.

And as of turning the sound on/off, I use rightclick -> remove/replace tab sound

It just looks like a muted tab, I cannot screenshot that it's playing sound. I feel like a screenshot is pointless. It happens randomly to me, it happened recently on a flash game at Kongregate.com and I think it happened on Youtube too.

I have the same problem, but it does not seem to be straightforward to reproduce. When I figure out a reliable scenario I will write it there. I invite other people to do the same, since a screenshot is of no value here (the only problem is about audio, nothing wrong on a graphical level).
