How are saving the file? When you use the right-click > Save [object] as... menu item, you should always get a file browsing dialog. But if the download is pushed from the server, then your settings come into play.

The "Always ask you where to save files" should work hand in hand with the "Save file" option:

Is that part not working?

On the other hand, Firefox does not ask if you choose either "Use [relevant application]" on the Settings page or "Open with [relevant application]" in the popup dialog with Open/Save/Cancel as the options. Firefox will always use the "Save files to" location, or, in Firefox 102+, you can set Firefox to use the Windows Temp folder for those.
