After upgrading to FF5 the appearance of the top of the window remained unchanged, no orange Firebox button appeared. Tried the Help and followed instruction to right-click on toolbar space and uncheck Menu Bar, but the orange button still not visible (Menu Bar becomes hidden as expected, though). No other symptoms.

You're right - I was running WindowBlinds themes manager; turning it off brings up the Firefox button.
