This new toolbar appeared about the same time I updated Firefox. It puts up a dialog box asking for personal information every time I open Firefox -- I don't care to have one more (unknown) entity tracking my behavior, but I can't see how to get rid of it. So far I just "X" out the dialog box but have to do this every time I open Firefox.

Hi squilici-

You should be able to remove the program in the Add-ons Manager. You can get more information and steps in the article on Uninstalling add-ons.

It starts with going into Firefox > Tools > Add-ons, which will open the Add-ons management.

Hope that helps.
