What happened to my "Close Tab" button?

All of a sudden I can no longer close a Firefox window by using the "Close Tab" button normally found under the File menu in the task bar. It just seems to have disappeared! If I have 2 windows open at the same time, I can't close just one of them. Instead, the only option I now have is to use the CLOSE button (red X), which then immediately closes BOTH my windows! Very annoying.

press CTRL + W to Close One Tab

There should be a X on the right side of tab by default to close tab. You can also use Middle-click (scroll wheel) on mouse or right-click tab and select "Close Tab".

You can open the File menu via Alt +F to see the Close Tab entry.

Some menu entries in the main menu bar are hidden if you use the mouse and only appear if you use the keyboard to open the menu (Bug 626825).
You can see the difference if you use Alt+F to open the File menu or Alt+B to open the Bookmarks menu and compare that to what you see if you use the mouse to open the menu after you have made the menu bar visible by tapping Alt or by pressing F10.

press CTRL + W to close the tab that you've selected. Simple, isn't it?
