I think you need a CSV file format. You normally get these by placing the data into a spreadsheet and exporting it to a CSV file, though whilst the 'C' stands for "comma" ISTR that a tab can work too. A spreadsheet program is handy because it lets you arrange the columns into a suitable order and eliminate unwanted or empty columns. If you don't have one installed e.g. Excel, then you can get a free one via Open Office or Libre Office, and Google do a reasonably competent online spreadsheet.
