Hi WalterGR, for me, Firefox 40 works the same as previous versions. One click adds a bookmark in the Unsorted bookmarks folder (you see the animation of the star going into the list). A second click opens the Edit Bookmark panel. Ctrl+d also is working normally.

Could you test in Firefox's Safe Mode? That's a standard diagnostic tool to deactivate extensions and some advanced features of Firefox. More info: Diagnose Firefox issues using Troubleshoot Mode.

If Firefox is not running: Hold down the Shift key when starting Firefox.

If Firefox is running: You can restart Firefox in Safe Mode using either:

  • "3-bar" menu button > "?" button > Restart with Add-ons Disabled
  • Help menu > Restart with Add-ons Disabled

and OK the restart.

Both scenarios: A small dialog should appear. Click "Start in Safe Mode" (not Refresh).

Any difference?

The functionality works when the add-on Tree Style Tabs is disabled.

How do I go about figuring out whether this is a bug in the add-on or a regression in Firefox itself?

Hi WalterGR, I suspect there was a change in Firefox that requires an update to Tree Style Tabs. The author of the extension probably has the most insight into the situation. However, if you want to research it further, this article might be a good starting point:
